Eligibility
You do not apply to the state. Each coordinated care organization runs its own SHARE spending process and must direct funds to social-determinants partners and involve its community advisory council. Researchers and universities normally enter as a partner to a community organisation or to the CCO, not as the fundholder. Spending must match the CCO's community health improvement plan.
