Social work and behavioral health expertise is named among the accepted backgrounds, making this one of the few national fellowships that explicitly opens a federal policy channel to social work researchers.

- Applicants must be U.S. citizens or permanent residents at the time of application. Midcareer is defined by experience, not age — typically at least the equivalent of 10 years of full-time practice or work experience — and candidates are expected to remain committed to health policy for at least 10 years afterward. Candidates come from academic institutions, not-for-profit organizations, or government; 'health services and social work/behavioral health' is explicitly listed among eligible expertise. Candidates may apply with a sponsoring institution or, under Track 2, without one.
