A three-phase prize competition to scale advanced community care networks led by community care hubs that integrate health care and community services for people dually eligible for Medicare and Medicaid, with measurable implementation progress required at each phase.

- Entrants must be a partnership between an advanced community care network and at least one health care organization, with commitment letters from both. The entity must be U.S.-incorporated and the team lead a U.S. citizen or permanent resident. Phases 2 and 3 are open only to winners of the preceding phase.
